Back to School Forms

At the start of every school year Saint Ann Catholic School has a variety of Back to School forms required for your student(s). Below are the various forms that need to be filed out. Once your forms are complete, you can use the web form below to submit the scanned / image of the completed form. You are also welcome to turn your forms into the office. Please note some of the forms can be filled out one per family while others are required one per student.

Also listed is a Back to School Forms Checklist to help you keep track of which forms you have completed and or submitted. If you require paper copies of the Back to School forms please inform the office, stann@stann.org. Please submit/return the Back to School forms by September 1st.

• Parent/Student Handbook Agreement Form  – Elementary School Families – parents need to sign one. Middle School Families must sign one per family and middle school student(s).
